Clinical Pharmacist Population Health

Coordinates pharmacy initiatives involving registry or high-risk patients, develops and implements medication management projects, and promotes high value, safe, and cost-efficient medication therapy in a healthcare network.

Location: Remote in Pennsylvania

Responsibilities

• Applies expertise in clinical pharmacy, therapeutic alternatives, utilization management, medication adherence, and Medication Therapy Management to improve outcomes in value-based reimbursement programs.
• Critically reviews drug utilization data and assures ongoing performance improvement of pharmaceutical care services.
• Evaluates patient EMR profiles for medication appropriateness, conducts patient appointments, and documents drug therapy recommendations.
• Identifies prescribing patterns that may lead to improper drug use and implements strategies to resolve cost barriers.
• Interacts with patients, providers, and pharmacy to impact CMS STARS, PQA, or HEDIS quality measures.
• Works with Population Health Director and ICC Pharmacy Manager on strategic pharmacy plans.
• Provides education to providers and staff through presentations, newsletters, and one-on-one interactions.
• Ensures quality of care through formulary and medication management projects.

Requirements

• Bachelor’s Degree in Pharmacy or Doctorate in Pharmacy.
• 2 years relevant pharmacist experience in patient care or population health or 1 year PGY-1 Residency.
• Skilled in critical appraisal of medical literature.
• Capable of independently reaching therapeutic decisions and communicating with healthcare professionals.
• Ability to monitor medications accurately.
• Ability to make independent judgments under stress.
• Experience in performance improvement projects related to medication management.
• Effective team working skills.
• Knowledge of laws, regulations, and standards related to medication management.
• Registered Pharmacist (RP) in Pennsylvania.
