Ambulatory Clinical Pharmacist

Provide integrated support to patients and physicians by developing personalized relationships to improve medication adherence and promote evidence-based care across various disease states.

Location: Colorado, USA

Salary: $127,000–$133,000 plus bonus

Responsibilities

• Provide medication management including reconciliation, dose modification, and adherence counseling within approved protocols.
• Collaborate with care team to develop individualized care plans based on evidence-based guidelines.
• Monitor and evaluate medication regimens for therapeutic goals, adverse effects, and interactions.
• Identify opportunities for drug therapy optimization and cost savings.
• Participate in quality improvement initiatives, clinical outcome studies, and process improvements.
• Support workflow optimization and operational changes to enhance efficiency and quality.
• Collaborate with healthcare providers for seamless medication management.
• Support peer-to-peer requests.
• Perform clinical assessments and educational counseling on targeted medications.
• Ensure clinical compliance with FDA, guidelines, and evidence-based literature.
• Promote patient safety and medication literacy.
• Maintain confidentiality of organization’s information.
• Participate in educational and professional development activities.
• Stay updated with advancements in therapy management.
• Perform other duties as assigned.

Requirements

• Doctor of Pharmacy (PharmD) from an accredited school.
• Licensed pharmacist in Colorado.
