VP Population Health

As Vice President of Population Health, you will lead and scale Covera's Protect Seniors program, a core pillar of the company's AI-driven quality platform. This role involves overseeing performance and growth of the population health model, managing key relationships with health plan and PCP partners, and collaborating with executive leadership to drive strategy and outcomes.

Responsibilities

• Lead and scale the Protect Seniors program, a core part of Covera's AI-driven quality platform.
• Own the performance and growth of the population health model.
• Serve as the primary relationship owner for health plan and PCP partners.
• Partner with the Executive Leadership Team to drive strategy, execution, and outcomes.

Requirements

• Experience in population health management, healthcare, or related fields.
• Strong leadership and strategic planning skills.
• Experience managing relationships with healthcare payers and providers.
• Ability to operate at the intersection of Customer Success and Operations.
