The Care Coordination Representative handles tasks related to Care Coordination services and Health Information Management, including electronic filing, order management, insurance verifications, and patient forms. The role supports accuracy, efficiency, and compliance in a healthcare setting, with flexible work arrangements.
Location: Remote
Responsibilities
- Process assigned tasks with accuracy, efficiency, and compliance.
- Evaluate medical documents and file them electronically into patient charts.
- Navigate client electronic health records (EHR) software and internal systems.
- Handle high volumes of work with high quality and accuracy.
- Utilize strong data entry and typing skills.
- Crosstrain in multiple EHRs for team coverage.
- Follow filing processes and consult documentation or supervisors as needed.
- Understand and utilize client protocols and Service Level Agreements.
- Track work to log time and transaction counts.
- Know the Planning Guidelines (PGLs) or Target Transactions per Hour.
- Complete all work assigned by management.
- Report variances and issues to management.
- Reference internal documentation and protocols.
- Maintain confidentiality and follow HIPAA guidelines.
- Provide exemplary customer service and professional communication.
- Attend meetings and training as required.
- Comply with compliance standards.
- Protect sensitive data.
- Perform other duties as assigned.
Requirements
- High School Diploma or equivalent.
- Experience with health information management, EHRs, or medical terminology is a plus.
- Proficient in managing high-volume, fast-paced environments with accuracy.
- Intermediate computer skills, including Microsoft Office and company-specific software.
- Able to navigate multiple programs and applications.
- Strong attention to detail and accuracy.
- Ability to perform physical tasks and lift up to 25 lbs.
- Occasional after-hours or weekend work.
- Reliable internet and private workspace for remote work.
