Personal Health Nurse - Remote

The Personal Health Nurse (PHN) at CCCS works within a team to manage members' medical care, focusing on quality and cost-effective healthcare through assessment, planning, coordination, education, and empowerment. The role involves collaboration with healthcare providers and interface with clients and managers, practicing within licensure scope.

Location: Remote

Salary: $64,168.00 - $96,262.00 annually

Responsibilities

• Manage accounts using the 4-Prong approach and Conifer Value Based Care tools.
• Review daily calendar for new episodes and tasks, prioritize accordingly.
• Complete Personal Health Management assessments and develop individualized care plans.
• Engage members, families, and physicians in the health management process.
• Collaborate with primary care providers and close episodes based on impactability and other criteria.

Requirements

• RN (Registered Nurse) license required.
• Bachelor’s degree in a health-related field.
• Three to five years of diverse clinical experience.
• Five years of UM/CM experience preferred.
• CCM certification preferred.
