NBHWC Certified Health Coach

Cadence is a clinical AI company providing proactive, continuous care for older adults with chronic conditions, integrating AI and clinical teams to improve patient engagement, clinician efficiency, and Medicare savings.

Location: Remote (US)

Salary: $22–$26 per hour

Responsibilities

• Conduct outbound telephonic patient outreach and provide coaching and support for chronic condition management.
• Help patients execute personalized care plans, providing education and coaching on behavior modification, nutrition, physical activity, and self-management.
• Conduct comprehensive health assessments and evaluate lifestyle, nutritional habits, and readiness to change.
• Educate patients on disease management, medication adherence, symptoms, and prevention, following clinical policies and protocols.
• Monitor patient progress and health outcomes through check-ins and remote tools.
• Document interactions and outcomes in the platform, identify barriers, and connect patients with resources.
• Collaborate with clinicians and support teams using various tools and contribute feedback for workflow improvements.

Requirements

• Active NBHWC health coaching certification.
• At least 1 year of experience as a health coach.
• Experience supporting patients with chronic conditions.
• Remote work readiness with stable high-speed WiFi.
• Fluency in Spanish (strongly preferred).
• Experience with AI-assisted clinical tools and ability to discuss AI support with providers and patients.
