Integrated Behavioral Health Clinician Embedded in Family Medicine Practice

LifeStance Health is a national provider of mental healthcare services focused on evidence-based, medically driven treatment for children, adolescents, and adults in outpatient settings, both in-person and via telemedicine. They employ a multidisciplinary team including psychiatrists, psychologists, nurse practitioners, and licensed therapists.

Location: Remote (IL)

Salary: $72,000–$90,000

Responsibilities

• Utilize evidence-based therapeutic interventions such as CBT, PST, and MI with patients aged 12 and above.
• Conduct comprehensive behavioral health assessments and communicate findings to the care team.
• Develop and implement individualized treatment plans addressing behavioral, physical health, and social needs.
• Collaborate with multidisciplinary care teams including PCPs, nurses, psychiatrists, and other healthcare professionals.
• Communicate behavioral health principles to non-behavioral health professionals to facilitate treatment planning and care coordination.

Requirements

• Licensed therapist (LCSW, LCPC, LMFT) licensed to practice in Illinois.
• Ability to see patients aged 12 and above.
• Experience in fast-paced, multidisciplinary care settings, preferably integrated care or primary care.
• Proficiency with electronic health records and documentation skills.
• Strong interpersonal and communication skills.
• Passion for expanding access to quality care through innovative treatment approaches.
