Care Coordinator - LPN/LVN - Fully Remote

Empassion is a healthcare startup focused on improving the experience and lowering costs for seniors at end of life through palliative and hospice care. They serve payers and risk-bearing entities across 44 states, providing impactful care for a neglected population.

Location: Remote (US)

Salary: $45k–$50k

Responsibilities

• Educating patients by phone about palliative care and scheduling appointments.
• Managing clinical files and entering notes into systems.
• Connecting patients with palliative care providers.
• Answering patient questions about palliative care and the program.
• Documenting patient encounters and triaging patient needs.
• Reviewing follow-up to ensure high-quality care.
• Closing cases daily and balancing clinical triage with patient calls.

Requirements

• Minimum 3+ years as an LPN.
• Minimum 3+ years of reading and updating clinical notes.
• Minimum 1+ years of relevant work experience in a customer service call center.
• Able to receive calls from patients, caregivers, and providers.
• Strategic thinking skills.
• Enthusiasm for providing outstanding patient experience.
• Passion for healthcare service and high standards.
• Strong communication and written skills.
• Detail-oriented.
• Comfortable with technology and dynamic startup environment.
• Secure Wi-Fi connectivity.
• Spanish-speaking skills are a plus, not required.
