CA Licensed Clinician: ASW, LCSW, APCC or LPCC | Behavioral Health

The Collaborative Care Clinician I is an entry-level licensed behavioral health clinician providing evidence-based interventions within the Collaborative Care Model for pediatric and/or adult patients with mild to moderate symptoms.

Location: Remote (US)

Salary: Licensed Clinician requiring supervision: $56,707.60 - $90,843.43; Licensed Clinician that does not require supervision: $67,441.92 - $102,585.10; Proficiency in multiple languages with salary adjustment: 5%

Responsibilities

• Provides care to patients, customizing treatment plans.
• Supports behavioral health care coordination with primary care providers.
• Screens patients for behavioral health and substance use disorders.
• Monitors clinical symptoms and treatment side effects.
• Supports medication management and treatment adherence.
• Tracks patient outcomes and records encounters.
• Plans mutual self-management goals.
• Facilitates care plan changes.
• Documents patient progress and treatment recommendations.
• Completes relapse prevention plans.
• Educates patients about disorders and treatment options.
• Establishes care management plans.
• Participates in caseload and supervision sessions.
• Identifies social barriers and works with Care Navigation Partner.

Requirements

• Master’s Degree in Social Work, Counseling, or related field.
• Licensed in California as an associate clinician under supervision or as an independent clinician.
• Minimum 1-5 years experience as a licensed professional.
• High-speed internet connection.

• Available schedules in Pacific Time Zone: 10am-6pm, 11am-7pm, 12pm-8pm.
